warped Vinyl, crinkled, Please help

I am using holographic vinyl and crystals in a design. The holographic vinyl warped, puckered, crinkled???? I do not know the proper terminology. I end up heat pressing the vinyl more than once because of the stones, not sure what's going on? Maybe too hot? Too many presses? Please advise. Thank you.
