You can go to eBay to find a customizable store. Some manufacturers will provide this service. If you have a large quantity, the price will be very cheap.

There is nothing special about doing a print like that. In terms of process it is just a two location print, like a chest print and sleeve print.

Assuming you don't live in the middle of the tundra, there are probably local screen printers you can ask for a quote.
