Try http://boards.screenprinters.net in their classifieds and post your image to see if any of the printers there could do it.
Also, once you have 15 posts here at t-shirtforums, you can post a SERVICE REQUEST in our classifieds. I'm sure some of our members who are printers could do that type of work.
