Could be a couple of things.
1) pretreatment and/or ink not fully cured (think screen printing and what happens when plastisol is not cured)
2) bad mixture of pretreatment
3) Not enough pretreatment or too much. There is a window of acceptable pretreatment.

Make sure you cure the pre-treat under high pressure. If you are curing the final print on a heatpress then it needs light pressure, if you are curing by just resting the heatpress on the shirt like you can on a GT541 then your design will flake off (been there and done that
