Can someone guide me on how this design and print is made, talking about the 'worn out look'

The "worn out look" is created using a "distress" pattern in the artwork. So it is done during the design process. Any print method can be used. Screen printing is most common for a retail quality product. DTG or transfers are good alternatives, depending on what you're looking for.
I attached it in the first place, I don't know how it was removed, I don't intend to do the exact same design, just need to know how it was made.

Anyways I have attached it again, I think the method used it heat printing, but can you check again whether it's the artwork or something else that gives that worn out look, since it's heat transfer I can't imagine how heat transfer can give this effect, shouldn't the heat transfer paper itself have that worn out effect?

It's the design, not the print method, that determines the worn out look. Even with heat transfers, if the design is created with a distress pattern, it will have that worn out look.
Can you send me a PDF sample of such a design?
I'm confused what you're looking for. Tons of shirts/designs out there are created with a distressed look. You can find free distress patterns if you search for them. Add it as a layer on top of your artwork and it will be fine.

We use distressed overlays. Take the overlay, place it over your artwork, change the color if needed, and you're done. Here's a free one from Ryonet Ryonet's Screen Printing Blog | free vintage distressed pattern effect screen printing

Tons more by googling 'distressed overlay'.
that shirt was done with screen printing using plastisol ink. the distress is part of the graphic.
Mr Retro machine wash filters if you are lazy like me will give you the look you are after I think.
