I think Ann from @CoopersDesignCo is in Utah: Welcome to Cooper's Rugged Design! and does DTG (she needs to be listed in PrinterListings :))

There's also a company nearby in Colorado that does contract DTG called Colorado Timberline - Colorado Timberline
