First you would get the woven labels made by one of the many label vendors that are out there.
Then you would find a company that will do relabeling. Some companies have sort of high minimums for that type work.
For example, Continental Clothing in the UK with relabel their blanks with the woven tags you provide but I think they have a minimum order of 600 pieces. You may want to contact them to see if that has changed.
