In regard to belt printing

Hi all, can someone point me in the right direction? I'm wanting to design a shirt that has something that wraps around from the front to the side (under the arm perhaps) and/or around the back. I've tried to look around and I've come across the fact that I would need either belt printing or all over printing, but what I'm unsure of is how to design the artwork exactly. For the sake of example, say I was drawing a stick figure on the front of the shirt, and I wanted his arm to wrap around to the back. Would I just create a flat image with his arm X amount of inches longer? Thanks for any insight on this.
