How does a place like threadless or designbyhumans get art approved when they are doing so many pop culture shirts like Star Wars and other big name stuff?

The websites don't get the art approved. They allow users to submit whatever designs they want in real time. If they feel that a design infringes on existing IP, they will have them removed at that point. Otherwise, they allow it under the premise that the user - not the website - is accountable. The reality is, both are legally at risk. But it's up to the IP owner to take notice and take action.
