Potentially it could be that a base layer was over flashed. It looks on my phone like the light skin colour may have been used as a base (phone too small to check) and the top ink is falling away.
Either way, it appears to be a curing issue, as mentioned.
If that customer has any left unworn and unwashed, take it back to the printer for another curing and rewash to test.
