Filling Out Application For A Sellers Permit Help (CA)

Here are some parts where I'm not sure what to put down.

5. Business Trade Name/DBA (Do I put down a name even if it isn't registered with the state or city?)

10. Title (Does it mean something like Owner or something like Mr or Mrs.)

50 and 51. Projected Monthly Gross Sales and Projected Monthly Taxable Sales (What's a reasonable number to put for a currently small operation?)

63. Major California Based Suppliers (I get all my supplies and products from out of state, so is it fine to leave this blank?)

That's all I couldn't figure out so far. Thanks in advance.

50 and 51. Projected Monthly Gross Sales and Projected Monthly Taxable Sales (What's a reasonable number to put for a currently small operation?)
$1000 for both. This number will be used to determine if you have to make a deposit and also your frequency of filing. This amount is reasonable for a startup and will make your frequency annual and no deposit.

63. Major California Based Suppliers (I get all my supplies and products from out of state, so is it fine to leave this blank?)
State that suppliers are out of state and name a few.

For number 5, if you have filed a Fictitious Name Statement, or incorporated, it would be one of those names; otherwise you leave it blank.

I agree with the rest of binki's answers.
Now that I think about it, I would also like to sell at some local events here and there. Do I have to list those as "other selling locations"(Even though I might not find out about some of them till later)?

No, selling locations are permanent places of business (you know, like store locations or your house if you are home based). Festivals, fairs, farmers markets and the like don't count.
