Have you tried running the print head alignment utility?
Is it printing double image from left to right or top to bottom? If its left to right then it is the encoder strip but top to bottom will mean something is wrong with the encoder wheel. If you take the left panel off, opposite of the control panel, you will see the encoder wheel disk. Clean it and the sensor with an alcohol pad and make sure the wheel isn't rubbing on the sensor by moving the bed in and out manually with your hand to spin the wheel. Also the light on the sensor should pass through the greyish outside part of the wheel.
It took me 1 week to replace the full upper part of the printer. I had several other 2100 printer for replacement. I changed head cable too, but the issue is still the same. Horizontally shadowed image. :S
It's very annoying!!!
What else could cause this issue? I didn't change the printhead and the encoder wheel.

I changed the full upper part of the printer, including the carriage belt, carriage assy, carriage moving electric engine. Everything. :SReally looks like a printhead alignment issue...
Have you tried cleaning the printhead carriage belt? They tend to get gunk
on the gear and sometimes cause rainbows on the shirts as well.
I tried alignment from the driver, but not changed anything.
I have the adjusment software also, but i think I not found that menu point.
It's not a very used printhead.

If the humidity goes under 40%, I always start the humidifier. The printer in the house, I haven't got climatic utility for the summer heat, I always open the window.Judging from the nozzle check you posted I would agree that it appears the printhead is on it's way out and probably should be replaced. The age of the printhead matters less then how it is maintained and the environment (humidity and heat) it is kept in.

I met only clogged printheads, but it's not clogged (the nozzle check is not shadowed, only the text under it).
I saw R2100 printer that many years old, and it's working. Usually these printers not used for a long time ago, but working! Peoples not maintain them, and they're working.
I don't know, but it's very strange...
