Custom one-off shirts?

Does anyone have any thought on where I could get a couple of one-off colorblock t-shirts (think of a tshirt with UCLA striping) or one-off baseball style jerseys? I'm in Ohio, but open to working with anyone in the US or Canada. I'm quite particular, but willing to provide incredibly detailed specs and drawings.
