Need advice on how to approach this print, on black.

5 color spot print (4 + solid white circle and halftoned white underbase)

Sorry, this image isn't a perfect mock-up.

It's going to be printed on a black shirt, and I was going to just leave the holes (which are white on here) as a drop-out that woudl be the color of the shirt

OK. Same suggestion just without the solid white area for the dot.
