SanMar's goes up to 4X in two adult styles, but they don't have any cotton if that's what you're looking for:

S&S has three options that go up to 3X:
Badger - B-Dry Sleeveless T-Shirt - 4130 (https://www.ssactivewear.com/ShopNow/Item.aspx?ID=1698)
Hanes - Original Tank Top - 4552 (https://www.ssactivewear.com/ShopNow/Item.aspx?ID=1957)
JERZEES - HiDENSI-T Sleeveless T-Shirt - 49M (https://www.ssactivewear.com/ShopNow/Item.aspx?ID=195).
These are also available from other distributors, too. I just saw them here.
Badger Sport's Core Sleeveless Tee goes up to 4X: (https://www.badgersport.com/Products/Item.aspx?ID=90). You'll probably have to get it from one of their distributors, but they can most likely drop ship it if they don't stock it.
