Seamstress wanted to create custom t-shirt

I am a newbie and ever since landing on this forum, I have become fascinated with all the information out there on just t-shirts! I now would really love to have t-shirts custom tailored for myself and from what I understand I need a seamstress to do so. I have a sample t-shirt that fits very well, the material is what I'd like my new t-shirts to have too. The only thing is, I want a modification done to the length of the sleeve (making it longer). Is there a simpler way of getting a longer sleeve? If not, I would not mind paying a seamstress to create a completely new t-shirt.

In which case, how do I go about my next steps? I'm in Los Angeles, CA.
