Assuming "with name" means "full legal company name" and not a shortened trade name, then I think that covers it. If it's a trade name, you'll need an RN. I also might be forgetting something... I'm going from memory after all, so I'd certainly recommend that you take another look at the regulations (either from posts in the FAQ here or directly on the FTC's website) if you haven't already.

I did design it to the FTC specs, but sometimes they get confusing. I am going to put my full legal company name at the top of the front side.
