Do I have to make the Image the exact size I want it to be on the shirt , Or does the person who does the serigraphy change that?

You can make it the exact size or larger, but you can't make the image smaller.

The person that handles the screen printing can usually have their art department resize the image to the size you want if you have given them a large enough starting image. If your design is vector, then it can be scaled to any size.
