Yeah most of the results I got are people who just embellish the cap with screen-print or embroidery. You need somebody who can manufacture the whole thing.
I found these guys:

They claim to be able to "customize to fit your needs" and they are in WI.
Take a look at
Alibaba.com - The world's largest online B2B marketplace. and use 'custom beanie' as the keyword. A lot of these guys are overseas and have pretty high minimums ... but it doesn't hurt to look ...
