Default Correct wording for copyright protection

Hello all,
I will be doing some heat transfer printing for a large group who will be submitting their own artwork. I may have 25 or so different designs. I need to know what is the correct wording I should have on my order forms to protect me if someone gives me some artwork that is not their own. I want the person giving the artwork to be responsible for the copyright.
