why did my transfer peel off on the first wash?

Default why did my transfer peel off on the first wash?

I finally got my transfers and shirts and finally fired up my press and made my own shirts.

I washed one of the shirts today just to see how it would come out.
The design on the front is fine but the tag washed off what is the possible problem?

I think we will need the type transfer paper/ink/press and temp/time. Not sure I would know the answer but some of our member surely will if they have the above information

I get my transfers made through red rocket graphics they are plastisol transfers. The temp is 390 for 10 seconds.

I think what may have happened is that there was a wrinkle or an air pocket under the transfer somehow and that's why it didn't stick.
