Per colour. With screenprinting each colour plate needs to be printed separately, regardless of what kind of source file is used to make the screens.
300dpi is the standard recommended resolution, and a good one since it will allow you to also use the design for print, re-size it a little, etc. but lower will print fine (I've printed 200dpi for example, and you could likely go a little lower) if that ever becomes relevant.

Yes. There are, however, legal requirements regarding what appears on the tag - so you'll have to make sure your new label covers that.

There are different types of screenprinting ink, the main two being plastisol and waterbased. Waterbased has a softer hand (feel on the fabric) than plastisol, though plastisol can also be modified to make it softer than it normally is. There's also discharge printing, which removes the dye from the shirt (and can then print pigment over the top). Discharge and waterbased printing will give you the softest results.
