First of all, I am not sure if this is in the right place, and if not then I apologize.
I love this forum. I have been reading it for about a week or two, so decided to sign up and become a member.
Let me state this... I hate asking the question that I am about to ask, because it will sound like I am trying to "mimick" another line, and that is not the case. This is just the easiest way for me to ask so everyone can understand.
I need to find the most cost effective way to create a shirts similar to Affliction, Ed Hardy, etc. My line is a Christian based line, will include crazy detail and graphics. I am looking at different washes (enzyme wash, tea wash, lava wash, etc), cut out patches, screen print, foil, etc. Obviously not ALL of the features on every shirt, but each shirt will probably include different ones.
I am looking at Bare Apparel for the shirts themselves. A little costly, but from my research, that is who Affliction uses, and its the quality that we are looking for. We are researching different print shops for the all over pint, and/or jumbo print, etc. i found 2 in columbus ohio that seem to be reasonable.
we are also looking for shops for custom tags, patch cut outs, etc. i need the cheapest possible price, but i cant afford to loose the quality.

You seem to be doing a decent job of research, so I'm not exactly sure what your question is.
But I will say this, Affliction, Ed Hardy, etc, are very high end shirts with all of the bells and whistles that you mention above. Trying to do that cheaply is like walking into a Ferrari dealership and asking for the cheapest one - even the cheapest one is still very expensive. So you're only going to achieve that level of quality by paying to do it right.
Just keep contacting various suppliers, printers, etc, and compare prices. But in all likelihood, you'll need to do high volume production in order to get the costs you're looking for.

Bare Apparel is very pricey. JS Apparel has a similar feel of shirt. They will grind the edges of the shirt for $1 more (minimum of 300, I think).
JS Apparel is around $4 a shirt without the grinding!
Ed Hardy or Affliction may use Bare Apparel or JS Apparel type shirts, but I don't think they buy direct from the aforementioned wholesalers. They go direct to the source. I know JS uses 4 different sewing contractors in LA. Bare Apparel uses one of these four, because their shirts are the same.
