Need help writing consent form

I finally have my website up, but eventually want to take pictures of all my friends wearing my shirts. I want to protect myself and create some sort of consent form that they can sign that basicially will cover my butt if someone decides they don't want it there anymore.

What should be included in this form? How should it be worded?
