I'm in desperate need of a customer order approval form. I am tired of losing money over the he said ,she said . I just don't how the wording should be or the setup.

I find simple English works best, unless you get it drafted by a lawyer and are prepared to back it with legal action then it remains just a bit of paper.
Spell out that the customer has seen the design and approves the work, in words of one syllable and you can use it for arm-twisting but little else.
Examine why you are having so many disputes...it probably won't be the lack of a form.
