customer work order approvals

Default customer work order approvals

I have been given the task to come up with a work order for customers to sign to take responsibility for their orders. I can't seem to come up with something that is quick and the wording. Can someone please help, I have been doing this for days and I need something asap.

This e-mail contains a proof for your sign(s). You can open the attached files using Adobe Acrobat Reader 4.0 or higher or in your browser as a .JPG. Please be sure to proof all of the information on your order carefully, especially phone numbers, addresses, emails, and all spelling. Because of the large volume of orders we produce, it is the customer's responsibility to check for any spelling, grammar, or design errors in the proof. If an error is found after your order has been approved and printed, the cost associated with reprinting the order is your responsibility. We assume no liability for orders approved by the customer, or for errors in submitted designs.
If you have any questions or changes that you would like made to your order please call or email me at your earliest convenience. If there are no changes, please print, sign and fax the attached approval form to .....

All I have is a blank tshirt template with specifics for sizing and colors. I'm stumped and do appreciate everything !

Sorry, but it sounds like you're kind of lazy and want something already done and handed to you. Contact other local business' that do the same thing as you, and research what you need.

I am not lazy, I have no idea what is used by any other company. I have no idea how to do this.
