I use two, either a work for hire or a a release form to use image, Just deoends if I am paying them or not. Both found on MS Word Templates

work for hire..... set a payment (or free)..... might want to state how long you can use the images also.... like 5 years or something if needed....

might want to include that only you and the model can use the photos....... like an exclusive deal
