When designing the graphic/picture in photo shop or Illustrator CS3 what size should the graphic/picture be so it is ready to be put on a shirt?

We print DTG with the Brother and I created a custom page size of 14x16 so I know what my print area is. If you are doing left chests, we rarely go over 4" wide. On the back, we usually stay at 13" wide. It all depends on the graphic. Just know your max print area and design for that.
