Maybe you modify this to make your own form.
Decal_designs, that's great you are sharing. I took Ace Transfer's and typed up and adjusted to my needs in Microsoft Word and then converted to Adobe PDF document.
By the way, I have learned a lot from your posts here. They have taken me to other specialty boards that have really provided more technical and detailed information for my needs. Anyone reading, look for Decal_Designs posts...worth taking time to search and read.

I'm still very new to all of this too, and I seem to spend much more time reading about it than actually doing it. Everyone here has helped me a lot, and I try to help in return if I can. There is so much that I don't know yet.
I know this question may have been asked, but i'm not finding the correct answer. can anyone tell me where I can get a blank t-shirt order form with the front and back of a t-shirt on the form. I need to be able to e-mail it to a highschool. thanks and sorry if the question has already been answered.
