i'm having a photo shoot done for my new products...

i was wondering if anyone can help me locate generic waiver/release forms for the models, so that i will be able'to use the photographs of them as i see fit--specifically, the image/likeness of the models.

with that said, do i need a seperate waiver or release from the photographer, so that i may use the photos exclusively, or can i use the same form?

are there country specific forms? if so, i'm in canada.

i've done some checking around online for waivers and release forms and have found some--but i don't know exactly what i'm looking for. i thought perhaps some experienced folk might be able to help...

i'd appreciate any advice on this matter.

didnt find anything via a google search, but id also like to get my hands on some of these. maybe just a template and we can make our own?
