Hi all

Just wondering if anyone has a simple order form for team jerseys for the size, name of player and number of player.

Something easy for the customer to fill out and easy to follow for me as we make the shirts.

Jersey orders primarily. We are working with about 10 different leagues and are doing spirit wear and team orders for jerseys. I need forms for both. I am hoping for something that leagues can fill out (or us) with the order. We have leagues handing us sheets of paper with all of their different individual teams and we are expected to decipher which teams get which sizing and numbers. Etc. I will take anything at this point.

I made this in excel hope it helps. I could easily add more options if you need.
