Production Forms?!?!?!

Default Production Forms?!?!?!

What production forms have you used in the past or currently that help you log and decifer colors, qty. etc. I believe screen works is a software that sets these up for you to fill in as well as numbers your artwork accordingly, is this system any good? Currently I provide a production request form, but it is not as detailed or successful as I would like it to be so it doesn't end up doing what it should and I would like to maybe "borrow" someideas from other successful formats.

here is one we give to our contract clients that usually bring us note book paper for PO's. Its simple and to the point.

Thanks fluid, thats what I was looking for, it has a few things I will maybe use, I especially like the way you combined the locations in the "mark the spot" area. It's easier to havejust the two images then to have a seperate one for back, sleeves, napes etc. I however need to add more detail, for my production guys to follow, the end result is hopefully to keep me out of the production area, and them more independent, all of this while maintaingin quality and control.

I got ya now. Try this one.

This is what wee use. I add the art and change the shirt template to the shirt color and print in color for the art folders. This is set up for our 10-clr automatic which is why it has squeegee angles, etc. might better fit your needs.

Yeah, thats more what I was thinking, I am making one right now that I will be able to use for this same thing, I will post it back up here when I get done with it. thanks again.

Fluid, can I ask for the form you use for embroidery jobs so I can have a good idea what to include in my form? Am not using any right now.

Right now I am sending my embroidery and screenprinting out to be done. I need a form with diagrams of a shirt and a place to put my cost and additional information. Anybody have something like that?
