Re: Order forms with product images We use order forms for all of our school and for some corporate clothing and promotional items. Each item is pictured with a mock-up of the design, a short description, colors available, sizes available, and the price (and the variances for up-sizes). A pitch for personalization costs and locations is included somewhere on the form as well as larger pictures of the logos for reference. We have found a separate order blank filled out with the actual order information is handy for entering the information into a summary spreadsheet for the ordering of clothing and for production. It (the order blank) is also used when packaging the orders for delivery as a double-check on accuracy of packing and as a reference for the customer to remind them of what they ordered.
Once we settled on a format that seemed to streamline the whole process, all the order forms have the same basic look, although the fonts and placements may be changed, and of course, logo information is different. The basic order blank stays the same, but the placement of school name, etc. is varied to keep the production packets easily separated for the CSR and the production staff.
