A 5x5 is way too large for LC, max for LC is 4 x 3 larger than that would require the image to be lowered on the chest towards the torso.. and even that would not look good.
To convert the image to digitizing we would charge 90.00 to the customer with embroidery at 1.25/1000. If you are going with a 5x5 then estimate a conservative stitch count at 13,000, once again depending on the complexity of the design. If it is a design with a lot of fill stitches then double that number.
From experience too many fill stitches in a LC will result in "bunching" and will look good off the machine, but not after the customer washes it. We have a LC design with 23,000 stitches which requires a lot of backing/solvi and even then it folds up.
As for shirts a basic 2100 would run something like $14.00 and something with a better hand ie: 5562 more like 25.00. So the price (retail) is also a major factor on your prices. The size (youth) really does not matter, more so the quality.
Since we don't have enough information for a "true" pricing quote, and do not know your profit margin. Price cannot be provided, but a guess of $300.00 is well in line.
