how would i get a design like this done? it is 100% stitched. i would have to get it digitized i know.

can anyone suggest a website or a phone number i can call. someone that does the whole process?

I suggest checking with shops in your area. they will be your best source for information. I encourage you to post a ad in the classifieds on this forum too. ..... JB

Something like that is done before the shirt is put together on specialized machines.

Yup, that looks like it was done as a flat and then assembled. Most intricate embroidered caps you see now days are done the same way.
