Default Creating a price quoter form on my website

Hello,

I am looking to place a simple price quoting form on my website prices page. I am able to design the formula in excel but am not sure if I can convert that to java script or such to place it on my website.

This would be a very simple form that would ask these following questions to the client:

Garment Type (drop down list)
Color (white or colors)
Quantity (can use a quantity break selection then enter quantity to make it easier)
Ink Colors Front
Ink Colors Back
Ink Colors Other

From that information it would provide the client a quote based on our price sheet.

Does anyone have any ideas on how to do this? I have been looking and searching for over a year on and off.

lol well he has been working on that for about 6 years now it links right into our data base so we can place the order online pretty much

Did you have to hire someone who knows html or java script to make that?

The excel page was easy to make, i just don't know how to add in a drop down menu. Our prices do not change much what so ever and if they do i just need to change 1 to 2 cells of data and it is all fixed
