Have you tried cleaning the encoder strip and also sensor at the back of the print head. Since its in the same spot all the time that might be the culprit
If it's in the same place everytime, the clipart is probably your culprit. Try printing something else and see if you still get it. If so, do a few head cleanings, which you probably already have, and make sure the pad at the left hand side (I can't think of the name right now) is not too dirty. If this continues with different designs also, you may need a new print head. Also, check your ink levels, they may be a little too low.
Like Zhenjie said I would check your encoder strip to make sure its nice and clean (clear grayish strip running the width of the printer behind the carriage) also check your artwork

You should make it a weekly routine to clean your encoder strip. Always clean towards the spring, never away from it. You may also want to keep a new encoder strip and spring in house just in case. They're only about $12.
Yep I have found a couple of times the encoder strip can creat havoc in the way the printer works thats the first thing I always clean when my printer starts printing wierd. I also clean weekly as part of my regular maintenance.
