Q1: All my basic CafePress shops are organized from a basic shop here:
galesgifts.windhavenweb.com- Do you think that the difference between this and the CafePress premium shop would be that different for customers? There is that jump where they are sent to my basic shops that I'm wondering if turns people off
I think moving to a premium shop will give your shoppers a much easier to follow, more organized shopping experience.
Everything is in one store which is easy to navigate via a consistent sidebar navigation of all the sections instead of jumping from single store to single store and possibly getting confused.

Q2: I also have other shops NOT part of CafePress which I am thinking of doing more cross promotion of these from the site I mentioned in q1. I'm sure I couldn't do that on a CafePress premium site due to their terms. Do you think this might be worth keeping things as is (so I could promote my other sites through this one?) or not
I think the premium shop is worth it on its own outside of what else you'd be doing.
It depends on what you're selling, but there are a few ways of promoting the other products. You could keep a website like the one in Q1 that links out to both your cp premium shop and to your other sites selling your stuff in an organized manner.

Q3: I'd say like 99.9% of my sales are marketplace sales. So I'm wondering whether if I went premium whether I'd get many more shop sales or whether it would likely stay the same (considering I won't be promoting this any differently than I promoted my off-Cafepress site which pointed to my basic shops. If you think I'll get more sales from a premium shop, why?)
I don't think you'll necessarily get more sales from a premium shop. You will just have a more organized storefront for shoppers.
If most of your traffic is coming from CafePress, then maybe a Premium shop might not add much, since they are sending you the sales from the marketplace and the shoppers aren't coming in through your store.
Having the premium shop does allow you to ramp up your shop promotion in a more organized fashion. You may want to consider doing more marketing/advertising to get traffic to your storefront so you don't have to rely on sales from the marketplace alone.

Q4: As you can tell I have a lot of designs (all in different basic shops). How easy do they make the transfer from basic to premium when you have a lot of shops? Can you transfer items from your basic to premium shops, and if so, what happens to those basic shops (because I have some people linking to some of my basic shops directly, including some affiliate links).
They have a pretty easy to use import process that allows you to import basic shops as their own sections.
Why not check out the free Premium Shop trial and clickety click around to see how it works?
That will give you hands on experience and you'll get to read how it works in the prompts.

Q5: Is managing one premium shop much easier than managing a lot of basic shops? I imagine altering prices is easier? How about adding items--especially changing sizes on items? I heard there was a trick if you usually size your shirt designs at 8" in stead of 10" that you could do in premium (have a hidden section and replace the design). Any other differences in management I should be aware of?
Yes, it's easier to manage one premium vs dozens of basic shops in my opinion.
Lots of bulk change tools (for designs, descriptions, pricing, etc)
